Required enrollment documents
To enroll your child at Tutus Family Child Care, please provide the following documents:
Completed Enrollment Form – Includes child and parent/guardian information.
Proof of Age – Birth certificate or passport.
Immunization Records – Up-to-date vaccination history.
Health & Medical Forms – Any required medical forms, including allergies or special needs documentation.
Emergency Contact Information – List of authorized guardians and emergency contacts.
Parent/Guardian Identification – Driver’s license or other valid ID.
Signed Policies & Agreements – Acknowledgment of our childcare policies, tuition, and schedule.
